Isi Ikharebha Green (At-Large)

Isi Ikharebha Green is the President of Physicians CareConnection a volunteer-based charitable organization that helps coordinate care for vulnerable adults in Central Ohio. Ms. Green has had the opportunity to be in a leadership role for Physicians CareConnection since 2009. During this time, she has seen the transformation of non-profit organizations as they move to meet ever changing community conditions. Physicians CareConnection specializes in health and related social needs that directly influence the quality of life for people who are vulnerable. By providing patient centered care coordination, our goal is to make connections to resources for people so that they can improve their lives. PCC has achieved successful healthcare outcomes for high-risk populations through our unique model of patient centered care coordination.

Ms. Green is the President of the Physicians CareConnection which is an affiliate of the Columbus Medical Association and has 18 years of non-profit leadership. Her primary responsibility is leading the effort to coordinate health care services and social resources for people who are vulnerable in Central Ohio and share the value of care coordination through outcomes measurements and evaluation. Isi is serving her God-given purpose by helping people. Her favorite saying is “Leadership is an opportunity to serve.” She is a proud Alumni of The Ohio State University and earned both her Bachelor of Arts in Chemistry and Masters in Public Health from this great institution.
