HIO Friends and Partners,
It has been an inspiring and insightful 2023 at Health Impact Ohio! The theme “momentum” comes to mind as I reflect on the year, one that has shaped our organization in ways we couldn’t have imagined as it began. We’ve worked to continue fulfilling our mission of improving social drivers of health, health equity, access and quality through community engagement and partnership, multistakeholder training and coaching, data collection and integration, and strategy development and deployment. All with one goal in mind: eliminating health disparities and helping to achieve optimal health outcomes for all people in every community.
HIO continues to be a leader in statewide efforts to provide quality improvement coaching with medical practices across the state. As part of the statewide Heart Healthy Ohio project, our quality improvement team has provided successful coaching that has resulted improvement in controlling blood pressure in hypertensive minority patients from around 66% to 71% of patients in participating practices over a three-year period. As we head into 2024, HIO’s quality improvement work will expand into several new initiatives, including Ohio Department of Medicaid QI initiatives and a much-needed effort to address physician and care team burnout.
Our Central Ohio Pathways HUB program has continued to grow and thrive, gaining state and national recognition for the innovative approaches to community health worker (CHW) led care coordination. The HUB has engaged 15 care coordination agencies in nine Ohio counties with forty CHWs employed. CHWs in our HUB have, to date, served 5,756 individuals. These CHWs are not only educating, referring, and helping their clients navigate health, social and/or workforce systems but 93% of the time, an outcome of verified connections to care and services, is achieved. Not only does this mean that clients reach their goals of better overall health, but in most cases, these outcomes generate payment via Medicaid Managed Care Organizations or other payers, bringing financial sustainability to our community for addressing otherwise unmet needs. Within the HUB work, we are extremely proud to highlight that 98% of babies born to mothers working with HUB CHWs have babies born at a healthy birthweight and that to date, CHWs in the HUB have housed 161 individuals and families through their services.
HIO’s Education and Equity team continues to enrich the lives of future and current CHWs. Our CHW Certification Training Program has now facilitated 10 cohorts of CHW students of all ages, races, genders, geographies, and backgrounds since 2021. 191 statewide participants have graduated since inception of the program; 65 of those individuals have successfully attained CHW certification through the Ohio Board of Nursing (OBN) in 2023 alone. In 2022, we began piloting CHW Certification Training for high school students and with generous support from the Ohio Department of Education, the Ohio Department of Health and the City of Columbus in 2023, HIO expanded the number of high school students trained. Enabling a young, eager generation to be job-ready and connected with living-wage employment as Certified CHWs the moment they receive their high school diploma renews our commitment to this work with each graduation ceremony.
We’ve been thrilled to continue the “Community Health Workers for COVID Response and Resilient Communities (CCR)” with the Ohio Department of Health and Centers for Disease Control and Prevention. The amount of outreach, training and networking this opportunity has provided us to work with CHWs and the 12 Pathways Community HUBs across Ohio has been the impetus to new and exciting endeavors, including a statewide conference that welcomed 300 CHWs to Columbus for a one and a half day of learning and collaboration. The momentum and connections our CCR Team have made has allowed for over 450 CHWs across the state to engage in bi-monthly educational trainings on topics that they can take directly into the field with them.
We’ve also made great strides in shining a light on CHWs and the work that they do statewide. We are currently working with state partners to roll out the Center for CHW Excellence, which was created and funded at a total of $5 million over 2 years through the state of Ohio’s biennial budget process. The Center will highlight the CHW workforce and its needs in our state, which has grown exponentially over the last several years.
All this work would be impossible without the unwavering leadership, support and counsel of our Board of Directors, as well as our dedicated staff and consultant team, who make even the most challenging work a joy. Our team truly hopes that you enjoy this year’s annual report, as much as our team has enjoyed reflecting upon 2023. Quality Improvement Initiatives
Health Impact Ohio has supported several quality improvement initiatives throughout 2023 through practice coaching. We are proud to continue leading efforts in this work with our community partners under the guidance of our quality improvement team.

Funded by the Agency for Healthcare Research and Quality (AHRQ), HHOI is an extension of our successful statewide activities conducted with the Ohio Department of Medicaid and Ohio medical schools as part of Cardi-OH. The Heart Healthy Ohio Initiative brings together the state’s regional health improvement collaboratives, HIO being one, professional organizations and other partners to expand Cardi-OH’s reach. Through HHOI we are leading a heart healthy QI project focused on hypertension management, smoking cessation, and disparities in 50 diverse primary care practices. During the first 9 months of HHOI, we have achieved improvements in both BP control (from 65% to 70% of patients with BP<140/90 mmHg) and smoking cessation assessment (from 80% to 90%). For this CDC proposal, we will build on HHOI clinical interventions and work to link clinic and community resources to address social drivers of health.

Rates of those diagnosed with diabetes have doubled in the past 20 years. Models suggest 1 out of 3 people will develop the disease. HIO is proud to be working on a partnership with Ohio Department of Medicaid (ODM), Colleges of Medicine (CoM), and other Community-Based Organizations (CBOs) on a project that aims to eliminate disparities in A1c control by integrating the lived experience and needs of patients and family members in Quality Improvement (QI) interventions. QI “hubs” across the state are required to engage patients and CBOs with the long-term goal of community engagement to establish linkages to social care for Medicaid beneficiaries. This project will work to identify and align priorities on areas of focus that have high potential to advance health equity in diabetes outcomes, working with practices and patients to understand and address barriers to diabetes care. We look forward to engaging this work further and sharing insights and improvements along the way!

Joy in work is an essential resource for the improvement of health and health care. With increasing demands on time, resources, and energy it comes as no great surprise that healthcare professionals are experiencing high rates of stress and burnout. Burnout leads to lower levels of engagement, decreased productivity, and lower patient satisfaction to name just a few. The patient experience is impacted, as lower engagement levels are linked to decreased quality in patient care. At HIO, we are developing a quality improvement program that focuses on understanding what matters to individuals in health care and what challenges they face in the workplace. We will then work with health care practices to identify what a joyful workplace looks like and help to create their joyful space using The IHI Model for Improvement.

Community Health Worker Statewide Conference
Community Health Workers (CHWs), supervisors, and public health professionals from every corner of Ohio gathered in Columbus on September 8th, 2023, to celebrate their profession, learn from experts in public health, and take time to network and enjoy time together. Health Impact Ohio (HIO), in partnership with the Ohio Network of Certified Pathways Community HUBs (the Ohio Network of HUBs), hosted 200 CHWs for a full-day conference, and 140 for a pre-conference on September 7th. These events were supported with funding from sponsorship dollars from the Centers for Disease Control’s (CDC) “Community Health Workers for COVID Response and Resilient Communities (CCR) project, and HIO’s vendor sponsor, CareSource.
The conference was inspired by the well-known term in the CHW space, “nothing about us without us.” The aim was to provide a safe, fun, and productive space for CHWs to garner what they desired from a conference in both education and experiential aspects.

The Ohio School-Based Health Alliance (“Ohio Alliance”) was founded in 2021 as Ohio’s only statewide organization created to advance and support the sustainability and expansion of school-based health care. Since 2010, the organization has been designated as the state affiliate of the national School-Based Health Alliance.

Led, facilitated, and convened a state team to secure $15 million in school-based health center funding in the State Fiscal Year 2024-2025 budget
Secured $380,000 in national and state grant funding to support food access and tobacco/nicotine initiatives across 16 SBHC sites
Developed a network of more than 500 school-based health care stakeholders across the state
Convened nine (9) learning community and informational sessions for school-based health care partners
Disseminated resources, supported partner connections, and provided technical assistance to 32 SBHC providers and more than 40 school district partners

Partnership for Advancing Community Health
Since March 2020, HIO has been an establishing member of a group of Certified Pathways HUBs across Ohio that came together to combine efforts to combat the COVID-19 Pandemic in Ohio’s most marginalized and vulnerable communities. Since 2020, this group of 12 HUBs have met weekly, addressing issues facing HUBs and community health workers statewide. In 2023, the group formally became a 501c3 nonprofit, and are officially operating as the Partnership for Advancing Community Health (PACH). Collaboration amongst Ohio’s HUBs will mean a concerted effort to put community health workers and the communities they serve at the center of public health initiatives and policy.

Center for CHW Excellence
Health Impact Ohio (HIO) is excited to announce that the state of Ohio’s biennial budget for fiscal year 2024-2025 included a provision that created and funded the Ohio Center for Community Health Worker Excellence. Spearheaded by Health Impact Ohio, this public-private partnership will include Board of Directors, including CHWs, The Partnership for Advancing Community Health, The Ohio CHW Association, state departments, and other stakeholder organizations. This funding will enhance and highlight the CHW workforce in our state, which has grown exponentially over the last several years, and appropriated $2.5 million each year of the biennium.
